Speare Memorial Hospital has issued its annual request for proposals from qualifying organizations to its Community Health Grant Program.

Organizations eligible to apply must be:

  • Tax exempt under Section 501(c)(3) of the Internal Revenue Code.information and application for Speare Memorial Hospital Community Health Grant Program
  • Aligned closely with Speare’s primary service area including the towns of Alexandria, Ashland, Bridgewater, Bristol, Campton, Ellsworth, Groton, Hebron, Holderness, Lincoln, Plymouth, Rumney, Thornton, Warren, Waterville Valley, Wentworth and Woodstock.
  • Submitting a proposal aimed at helping our communities best achieve optimal health.
  • Making a grant request for a minimum of $2,000.

The deadline for grant proposals along with the required application is end-of-business on Monday, June 15. The funding period for Speare’s Community Health Grants is October 1, 2015 – September 30, 2016.

Speare’s community Health Grant Program was established to provide funding to organizations to help accomplish significant results in improving population health in our central New Hampshire communities.  Explains Speare’s President & CEO Michelle McEwen, FACHE,

“Speare Memorial Hospital has been our communities’ health resource for many generations, and we are committed to providing excellent healthcare to our community every day, in pursuit of each individual achieving optimal health.  This means not only providing care for the injured and ill, but helping individuals take an active role in managing their own health through education, prevention, wellness and primary care.  To that end, it takes the efforts of many organizations in our region working together toward that common goal.”

Each year Speare’s Board of Directors approves a pool of funds to support the Community Health Grant Program.  An appointed committee reviews all grant requests that address a community health need as identified in the Central New Hampshire Health Partnership Community Needs Assessment, also available online with the grant application.  Grant awards will be dependent upon the size of the pool and the number of grant applications received.
