Your physician will write a discharge order as soon as he/she decides you are well enough to go home. Your social worker can help make the necessary arrangements. Please discuss any special requirements with the social worker.
Your post hospital care will be discussed with you by your doctor and nurse and will include any special information needed with regard to diet, exercise, bathing, medications, and lifestyle. They will be happy to answer any specific questions you may have.
Check your room for personal belongings and remember to reclaim any items placed in the hospital safe prior to leaving.
A questionnaire about your hospital stay will be given to you as you are discharged. We hope you will take the time to fill it out and return it with any additional comments you may have about our staff or facility. Your thoughts and concerns are important to us. You may receive a follow-up phone call from our discharge planner to monitor your progress at home.
