Speare Memorial Hospital

Case Management

Discharge Planning
Our medical social workers/discharge planners are available to help meet your special needs during and after your hospital stay. Your hospitalization may be brief, but it can often magnify your social, emotional, and economic problems. Working in collaboration with the Discharge Planning Team , the Social Worker assists patients or families in many ways, including:

  • Support in coping with emotional stresses of injury or illness.
  • Planning and coordinating services the patient may need when leaving Speare Memorial Hospital or transferring to another facility.

If you have any questions about these services, please ask your physician or or nurse to contact Social Services.

Going Home
Your physician will write a discharge order as soon as he/she decides you are well enough to go home. Your social worker can help make the necessary arrangements. Please discuss any special requirements with the social worker.

Your post-hospital care will be discussed with you by your doctor and nurse and will include any special information needed with regard to diet, exercise, bathing, medications, and lifestyle. They will be happy to answer any specific questions you may have.

Check your room for personal belongings and remember to reclaim any items placed in the hospital safe or medications brought from home prior to leaving.

A questionnaire about your hospital stay will be given to you as you are discharged. We hope you will take the time to fill it out and return it with any additional comments you may have about our staff or facility. Your thoughts and concerns are important to us. You may receive a follow-up phone call from our  case manager to monitor your progress at home.

As you leave, our medical staff and employees wish to express our appreciation and wish you good health.
