For your comfort, you will want to have a few personal items, such as a bathrobe, night clothes, slippers and personal toiletries including things such as a toothbrush, toothpaste, deodorant, a brush or comb, etc. Please do not bring more than $5.00 for personal use. Please leave all jewelry and other valuables home.
Please provide or bring a list, including dosage and frequency of any medications that have been prescribed for you and that you are currently taking. This will help us better plan for your care. If you have a Medication Card, please bring that with you. Your Medication Card will be updated by your physician or nurse.
For Medicare Patients
If you are being admitted to an OUTPATIENT level of care, any medications that you take regularly by swallowing them, are an insulin dependent diabetic or use an inhaler, Medicare will not pay for any doses of these drugs which you receive from the hospital. You or a family member should bring your medications in their clearly labeled,original bottles. We can supply you with these medications but you will be financially responsible for paying for them at our regular prices.
You should bring any insurance cards, calm forms, or authorization forms with you.
