Achieving and maintaining excellence in patient care is a top priority for the staff of Speare Memorial Hospital. We are constantly monitoring the care we provide our patients with the goal of excellence in health care.
One of our hospital-wide performance improvement projects underway focuses on patient safety and the prevention of patient falls during hospitalization with the implementation of the “falling leaf" program. This program will assess patients for fall/injury potential and they will be identified as either a low or high risk for falls.
Appropriate interventions will be put in place by the nursing staff to ensure that the care and environment surrounding the patients promotes safety. One such intervention is identifying those patients at risk with leaf at the entryway to their room and on the patient chart, which will communicates to the entire hospital staff that this patient needs careful monitoring.
Current research shows that a hospital-wide commitment to patient safety is one of the most effective ways to prevent patient falls.
